Pri-Med Alternate Site Program

A Pharmacy Delivery Program designed to meet the pharmaceutical needs of practitioners at alternative care sites.

Pri-Med Infusion will be responsible for dispensing non patient specific pharmaceuticals as ordered by the practitioner and will deliver to office location or facility as directed.

New Accounts

If you have never placed an order with us, you must include with original order a copy of:

DEA number

License number

Orders

	Orders are requested 48 hours prior to the scheduled delivery time.
	Orders may be faxed 24 hours a day, 7days a week to (773) 467-6001.
	Confirmation calls to verify receipt of orders will be made during normal business hours Monday – Friday, 8:30 am – 5:00 pm.

Verbal orders will be accepted when necessary (e.g., fax machine unavailable or late changes in medication) and must be followed with signed written orders.

Payment

	Direct billing will be made to your facility or insurance company.
	Include any purchase order number information when placing the order.

Shipping and Delivery

Orders under $100.00 may require a $25.00 delivery charge.
